To request a pick-up of a used wheelchair, please complete the following on-line request form. Please remember to complete all the fields. List of Drop-Off Locations.

If you are located outside of New England, we do not have the ability to pick-up your wheelchair. If you still wish to donate a chair, please ship it to 45 Barber Pond Road, Bloomfield, CT 06002.
